SLA e Colesterolo cattivo
LDL alte , utili nella sclerosi amiotrofica laterale

La sclerosi amiotrofica laterale (ALS) o malattia di Lou Gehrig,
nome attribuito a questa patologia dopo la morte, per sua causa, del famoso giocatore di baseball è un disordine del sistema nervoso centrale che distrugge le cellule responsabili dei movimenti volontari. Ricercatori francesi hanno seguito 369 pazienti affetti da ALS e 286 soggetti sani scoprendo che i malati di ALS mostrano ipercolesterolemia rispetto ai controlli, in particolare i livelli di LDL sono doppi rispetto alla norma.
I malati di ALS con livelli di colesterolo HDL, ovvero quello buono, più alto di quello cattivo LDL, hanno un 35% di possibilità in più di morire:
l’aspettativa media di vita nei soggetti con elevati livelli di LDL è stimata in 49,2 mesi, contro i 36,7 stimati quando i livelli di LDL sono più bassi. Il significato di questa scoperta, purtroppo, non è ancora chiaro, ma già esperimenti precedenti condotti sui topi avevano messo in luce questa stessa strana relazione. -->Tags: ALS LDL HDL Fonte: Reuters Health

Inasmuch as lipids are the major source of energy for muscles, we determined the status of lipids in a population of patients with ALS and investigated whether lipid contents may have an impact on disease progression and survival.
METHODS:
Blood concentrations of triglycerides, cholesterol, low-density lipoprotein (LDL), and high-density lipoprotein (HDL) were measured in a cohort of 369 patients with ALS and compared to a control group of 286 healthy subjects. Postmortem histologic examination was performed on liver specimens from 59 other patients with ALS and 16 patients with Parkinson disease (PD).
RESULTS:
The frequency of hyperlipidemia, as revealed by increased plasma levels of total cholesterol or LDL, was twofold higher in patients with ALS than in control subjects. As a result, steatosis of the liver was more pronounced in patients with ALS than in patients with PD. Correlation studies demonstrated that bearing an abnormally elevated LDL/HDL ratio significantly increased survival by more than 12 months.
CONCLUSIONS:
Hyperlipidemia is a significant prognostic factor for survival of patients with amyotrophic lateral sclerosis. This finding highlights the importance of nutritional intervention strategies on disease progression and claims our attention when treating these patients with lipid-lowering drugs.

Per tutti quelli che hanno partecipato al convegno,
e hanno assistito a delle affermazioni importanti da parte di alcuni relatori..
volevo sottolineare che non mi trovano assolutamente d'accordo e mi lasciano molto perplesso oltre che riflettere su alcuni punti da chiarire a tutti voi:
Di seguito elenco le principali affermazioni :
1) Da parte dell'ASL di BG l' affermazioni è stata:
il ministro Livia Turco ad Agosto 2007 ha finanziato i soldi alle Regioni per l'aquisto dei comunicatori vocali, alla Regione Lombardia sono andati circa 1.600.000,00 euro ma non sappiamo che fine hanno fatto questi soldi in Regione......
Ritengo che questa affermazione se vera...
necessiti quanto meno una denuncia da parte delle stesse autorità che la riportano e poi non può essere usata a scopo giustificativo in un convegno dove i malati e i loro parenti cercano una speranza e non delle polemiche
2) E' stato detto dal Dr B.V. che è meglio che questi soldi non siano ancora arrivati perchè finirebbero con spenderli male in quanto non sono preparati a prescrivere i comunicatori.
Altra affermazione che mi lascia allibito:
per tanti motivi primo fra tutti non trovo così difficile la prescrizione di un comunicatore sulla base di una funzionalità residua del paziente e poi mi chiedo se non è pronto a questa banale prescrizione a che cosa sarà mai pronto come referente medico dei malati di SLA...
3) La terza affermazione sempre dalla stessa voce del punto due è stata quella rivolta ai malati di SLA che hanno chiesto tramite ricorso in tribunale di avere l'IGF-1BP3, dicendo che stanno rubando soldi alla ricerca usufrendo di questo farmaco che costa 140 mila euro all'anno.
Scioccante affermazione che non mi vede d'accordo in quanto i soldi per l' IGF-1 vengono presi da un fondo destinato alle malattie rare, comunque questi soldi non sarebbero' dati in alcun modo alla ricerca,
inoltre ritengo insindacabile il diritto di un malato di curarsi come ritiene meglio per se stesso in termini di legge.

con particolare attenzione
all' effetto di tossici e inquinanti ambientali e/ o agenti patogeni infettivi,
non escludo in sinergismo a cofattori non meno importanti quali:
stress cronico, stati ansiosi- depressivi trascurati:
Se avessi la possibilità di eseguire una raccolta anamnestica di ogni malato di SLA
potrei provare a vedere se questa
correlazione è solo una ipotesi o trova fondamento nella mia piccola esperienza,
purtroppo non esiste una informattizzazione dei dati anamnestici dei pazienti e non ho ancora trovato un interesse in chi potrebbe raccogliere questi dati.

Vi lascio qulche informazione a riguardo l'antibiotico di cui si parla:
Clindamicina

La clindamicina è attiva sui cocchi Gram positivi, compresi gli stafilococchi resistenti alla penicillina, e anche su molti anaerobi, soprattutto Bacteroides fragilis. Si concentra bene nell’osso ed è escreta nella bile e nelle urine.

La clindamicina è consigliata per le infezioni stafilococciche dell’osso e delle articolazioni, come l’osteomielite, e per le sepsi intraddominali; rappresenta un'alternativa ai macrolidi per la erisipela o la cellulite nei pazienti allergici alla penicillina. La clindamicina è anche indicata per le infezioni causate dallo Staphylococcus aureus resistente alla meticillina nelle infezioni delle bronchiectasie, nelle infezioni dell'osso e delle articolazioni, nelle infezioni della cute e dei tessuti molli.

E' usata anche per la profilassi dell’endocardite in pazienti allergici alla penicillina Prevenzione dell’endocardite in pazienti con valvulopatie, difetti del setto, dotto arterioso pervio, protesi valvolare o storia di endocardite (indicazione non registrata in Gran Bretagna né in Italia).

La clindamicina ha un utilizzo limitato a causa dei suoi effetti indesiderati gravi. Il più pericoloso è la colite da antibiotici (Colite associata agli antibiotici) che può essere fatale ed è più frequente in donne di mezza età e anziane, soprattutto dopo interventi chirurgici.
Nonostante questa malattia possa manifestarsi con gran parte degli antibatterici è stata più spesso osservata con la clindamicina. Alla comparsa di diarrea i pazienti dovrebbero quindi interrompere subito il trattamento.

Infezioni del cavo orale
La clindamicina non deve essere usata di routine per le infezioni del cavo orale perché è meno efficace delle penicilline contro i germi anaerobi e ci può essere resistenza crociata nei batteri resistenti all’eritromicina. La clindamicina è indicata per il trattamento dell'ascesso dentoalveolare non responsivo a penicillina o metronidazolo.

Clindamicina
Indicazioni Clindamicina:
infezioni stafilococciche delle ossa e delle articolazioni, peritonite; profilassi dell’endocardite, Prevenzione dell’endocardite in pazienti con valvulopatie, difetti del setto, dotto arterioso pervio, protesi valvolare o storia di endocardite (indicazione non registrata).

Avvertenze
Sospendere subito in caso di diarrea o colite; insufficienza epatica o renale; controllare la funzionalità epatica e renale per terapie prolungate nei neonati e nei bambini; gravidanza; allattamento (Clindamicina); evitare la somministrazione endovenosa rapida.

Interazioni Clindamicina.
Controindicazioni Stati diarroici; nei neonati evitare le iniezioni contenenti alcol benzilico (controllare la lista degli eccipienti).

Effetti indesiderati
Diarrea (interrompere il trattamento), malessere addominale, esofagite, nausea, vomito, colite da antibiotici; ittero; segnalate leucopenia, eosinofilia, agranulocitosi e trombocitopenia; segnalati rash cutaneo, prurito, orticaria, reazioni anafilattoidi, sindrome di Stevens-Johnson, dermatite esfoliativa e vescicolo-bollosa; dolore, edema e ascessi dopo iniezione intramuscolare; tromboflebiti dopo iniezione endovenosa.

Posologia
Per via orale, 150-300 mg ogni 6 ore; fino a 450 mg ogni 6 ore nelle infezioni gravi; bambini: 3-6 mg/kg ogni 6 ore.

Si consiglia ai pazienti di sospendere subito il farmaco e contattare il medico in caso di diarrea; le capsule dovrebbero essere inghiottite con un bicchiere d’acqua.

Per iniezione intramuscolare profonda o infusione endovenosa, 0,6-2,7 g al giorno (in 2-4 dosi); nelle infezioni che mettono a rischio la sopravvivenza, fino a 4,8 g al giorno;
dosi singole oltre i 600 mg solo per infusione endovenosa;
le dosi singole per infusione endovenosa non devono superare 1,2 g.

Bambini: oltre 1 mese, 15-40 mg/kg al giorno in 3-4 dosi; nelle infezioni gravi, almeno 300 mg al giorno indipendentemente dal peso.

Autophagy, like the ubiquitin-proteasome system, is considered to play an important role in preventing the accumulation of abnormal proteins. Rat microtubule-associated protein 1 light chain 3 (LC3) is important for autophagy, and the conversion from LC3-I into LC3-II is accepted as a simple method for monitoring autophagy. We examined a SOD1G93A transgenic mouse model for amyotrophic lateral sclerosis (ALS) to consider a possible relationship between autophagy and ALS. In our study we analyzed LC3 and mammalian target of rapamycin (mTOR), a suppressor of autophagy, by immunoassays. The level of LC3-II, which is known to be correlated with the extent of autophagosome formation, was increased in SOD1G93A transgenic mice at symptomatic stage compared with non-transgenic or human wild-type SOD1 transgenic animals. Moreover, the ratio of phosphorylated mTOR/Ser2448 immunopositive motor neurons to total motor neurons was decreased in SOD1G93A-Tg mice. The present data show the possibility of increased autophagy in an animal model for ALS. And autophagy may be partially regulated by an mTOR signaling pathway in these animals.

The proteasomal and lysosomal/autophagy pathways in the liver and other tissues are involved in several biological processes including the degradation of misfolded proteins. Exposure of hepatocyte cell lines to proteasome inhibitors (PIs) results in the formation of inclusions that resemble Mallory-Denk bodies (MDBs). Keratins are essential for MDB formation and keratin 8 (K8)-overexpressing transgenic mice are predisposed to MDB formation. We tested the hypothesis that PIs induce MDBs in vivo and that autophagy participates in MDB turnover. The effect of the PI bortezomib (which is used to treat some malignancies) on MDB formation was tested in K8-overexpressing mice and in cultured cells. Inclusion formation was examined using immune and conventional electron microscopy (EM). Bortezomib induced MDB-like inclusions composed of keratins, ubiquitin, and p62 in cultured cells. Short-term exposure to bortezomib induced similar inclusions in K8-overexpressing but not in nontransgenic mice, without causing liver injury. In bortezomib-treated mice, autophagy was activated in hepatocytes as determined by EM and biochemical analysis. Further activation of autophagy by rapamycin (Rap) decreased the number of inclusions in bortezomib-treated K8 transgenic mice significantly. Rap also led to resorption of spontaneously formed MDBs in aging K8-overexpressing mice. Immune EM demonstrated K8-positive and ubiquitin-positive structures in autophagic vacuoles in the mouse liver. Conclusion: PIs alone are sufficient to induce MDBs in susceptible animals, while Rap-mediated activation of autophagy prevents MDB formation and causes MDB resorption. These findings suggest that some patients treated with PIs may become predisposed to MDB formation. Autophagy provides a potential cellular mechanism for the resorption of cytoplasmic inclusions. ALS is a devastating neurodegenerative disorder with no effective treatment. In the present study, we found that daily doses of lithium, leading to plasma levels ranging from 0.4 to 0.8 mEq/liter, delay disease progression in human patients affected by ALS. None of the patients treated with lithium died during the 15 months of the follow-up, and disease progression was markedly attenuated when compared with age-, disease duration-, and sex-matched control patients treated with riluzole for the same amount of time. In a parallel study on a genetic ALS animal model, the G93A mouse, we found a marked neuroprotection by lithium, which delayed disease onset and duration and augmented the life span. These effects were concomitant with activation of autophagy and an increase in the number of the mitochondria in motor neurons and suppressed reactive astrogliosis. Again, lithium reduced the slow necrosis characterized by mitochondrial vacuolization and increased the number of neurons counted in lamina VII that were severely affected in saline-treated G93A mice. After lithium administration in G93A mice, the number of these neurons was higher even when compared with saline-treated WT. All these mechanisms may contribute to the effects of lithium, and these results offer a promising perspective for the treatment of human patients affected by ALS.

In a pilot clinical study that we recently published we found that lithium administration slows the progression of Amyotrophic Lateral Sclerosis (ALS) in human patients. This clinical study was published in addition with basic (in vitro) and pre-clinical (in vivo) data demonstrating a defect of autophagy as a final common pathway in the genesis of ALS. In fact, lithium was used as an autophagy inducer. In detailing the protective effects of lithium we found for the first time that this drug stimulates the biogenesis of mitochondria in the central nervous system and, uniquely in the spinal cord, it induces neuronogenesis and neuronal differentiation. In particular, the effects induced by lithium can be summarized as follows: (i) the removal of altered mitochondria and protein aggregates; (ii) the biogenesis of well-structured mitochondria; (iii) the suppression of glial proliferation; (iv) the differentiation of newly formed neurons in the spinal cord towards a specific phenotype. In this addendum we focus on defective autophagy as a "leit motif" in ALS and the old and novel features of lithium which bridge autophagy activation to concomitant effects that may be useful for the treatment of a variety of neurodegenerative disorders. In particular, the biogenesis of mitochondria and the increase of calbindin D 28K-positive neurons, which are likely to support powerful neuroprotection towards autophagy failure, mitochondriopathy and neuronal loss in the spinal cord.
